Referral Process

 

 

The referral process at Phoenix Alternatives, Inc. (PAI) consists of  the following steps:

 

• IDT member contacts PAI and expresses interest in the PAI services offered with a tour to be arranged.

 

• After the tour, if the consumer and the IDT want to pursue enrollment, the consumer's legal representative requests the county case manager to submit an application and the required referral information to PAI.

 

• PAI will make arrangements to visit the consumer at their school, residence or current day program to complete an Intake Assessment.

 

• After the necessary preliminary information is received and the Intake Assessment has been completed, the Admission Committee at PAI reviews the information and determines if PAI is able to offer enrollment.

 

• After the Admission Committee has made determination, notification 

letters of acceptance or denial are sent to the consumer's IDT.

  • Current Individual Service Plan
  • Psychological
  • Signed Release of Information
